In the beginning of the Mulanay: Sa Pusod Ng Paraiso, the main character was highlighted, Dr. Ria Espinosa (Jaclyn Jose) as a newly listed doctor, decided to join and be destined in the Doctors to the Barrios. Where Dr. Espinosa was welcomed as VIP—It can be seen as, this can be the answer to lift this place higher, provide medical needs and ensure public health safety. Dr. Espinosa felt a culture shock from the difference between the city and the underdeveloped province of Mulanay, and the people in the community were completely unaware and had no idea how their community could be changed and the problem could not be addressed. This includes the no toilet and bath, limited source of energy at night through time she became one of them and took a bigger leap—fundraising to respond to the community’s deficiencies. 


It is evidently seen that the community is dependent on healing from prayers and their quack doctors. They believed that illnesses can be healed with the absence of medical credibility and proper practice. When Dr. Espinosa arrived, treating the wound—there was this parent who asked “why does the doctor not use the stethoscope?”— it pertains to the fact that people thought that a stethoscope is a healing agent for any illnesses. Only limited people believe what medicine can do, and some of them do not know what or who is DOH, they thought of it as a medicine brand but it is who provided the medicines, they think of it that medicine is expensive and they could not afford it. Dr. Espinosa as she understands the situation and belief more, she never argued against it—coolly said “I just had a different practice.” 


The illnesses kept spreading and growing due to the lack of clean water and sanitation and sick citizens continued to swamp as being innumerable. Dr. Espinosa could not accommodate them all, incomplete medical equipment and a small clinic, the citizens resorted to a quack doctor and to a priest. It concludes that the citizens continued to ignore someone who had proper practice because of their personal healing beliefs. There were these parents, who doubted the capabilities that the doctors can bring for the case of an ill-ed son, and this resulted in death due to a prolonged lack of medication. 


The distance of the Bayan and their community and traveling time and method reacts as a burden, adding also to the animal livelihood of the citizens. Dr. Espinosa played as a role model with disciplining each family about health and cleaning their environment, still, it is not enough, it needed collective action. Dr. Espinosa thought that providing deep cleaned wells will aid the community’s problem as she finds a fund—at this part, painting is more valuable than the condition of the barangay and it can support the community with just one piece of painting. Dr. Espinosa, out of an act of concern, continued to move and reach out to the Mayor even though it was out of her capabilities. After addressing it to the Mayor who has not full authority to provide it and However, Dr. Espinosa did not stop there, they had discussions together with the community, addressing each suliranin she stated that “mas malaki ang problemang ito kaysa sa’tin” she ended up setting up a small drug store. 


The movie is a wake-up call not only for their era and generation but until today. Almost 25 years had passed since this was released and when we look at it, the issue before up until this day has still not been rectified. It gives us a clearer perspective on how the Local Government Unit works, the Barangays depend on the LGU—must be always accountable, the Mayor awaits for the LGU provisions—there was no proper leadership. In the movie, it was evidently seen that the LGU lacked supervision of its constituents. LGU was supposed to address its community's problems and concerns, enforce policies and hold influence over its communities; to facilitate and maintain the public works, highways, social welfare, health, and agriculture. There was insufficient funding even though the Mayor states it is one of the priorities, it falls to be questionable. What are the priorities that should be done for this community?
